- That I am major and my actual date of birth is 21.10.1985. I am having Marksheet of Secondary School Examination issued by CBSE Board.
- That I have embraced Muslim Religion and converted my religion from Hindu to Islam with my free will and consent and without any force, pressure and coercion from any corner and without consuming any intoxicating substance.
